If you’re filing a paper state tax return, you need to file Form DR0104CH_2019 in order to donate to us (and thank you)!  You can download the form here: DR0104CH_2019_JCLF .

We’ve filled in our name and reg. # for you – all you have to do is enter the amount you want to donate on line 19 of the form and enter the amount of your refund from your state return on line 20, and submit the form with your tax return.
